Patient Samples – Important Information

Please follow these instructions when dropping off samples at the GP practice.
- Clinician request required: Samples are only accepted if requested by a GP or nurse. Unrequested samples may be rejected.
- Use the correct container: Only use the sample pot provided by the surgery or pharmacy. Different tests require different containers.
- Label clearly: Write your full name, date of birth, and date and time of collection on the pot. Include your NHS number if known.
- Package correctly: Place the sealed pot in the clear plastic bag provided. Keep the request form in the separate sleeve.
- Return promptly: Bring the sample back as soon as possible, ideally within 1 hour.
- Storage if delayed: Most samples can be stored in the fridge (4-8°C) for up to 24 hours, unless you are told otherwise.
- Hygiene: Wash your hands before and after collecting the sample.
Sample Collection
- Urine: Collect a mid-stream sample after washing hands and genital area.
- Stool: Transfer a small (walnut-sized) amount into the pot using the spoon provided.
